The enhanced Lewisham Homeless GP Service is commissioned by South East London Integrated Care Service (SEL ICS) to support the homeless population across Lewisham.
The aim of the service is to offers fast, easy to access 'wrap around', multidisciplinary care, support and monitoring to Homeless patients & Rough Sleepers as well as offering crisis support for the wider group of Homeless patients in Lewisham.
The service provides weekly GP clinics at the following sites and supports over 213 clients;
· Spring Gardens (hostel)
· Lewisham Assessment and Recover Centre (LARC) (hostel).
· Honor Lea (hostel).
· Deptford Reach – Day centre for rough sleepers and homeless
· Fairway Lodge – (hostel for rough sleepers) (Bi-weekly clinics)
The clinics offer both appointment and walk-in appointments and are delivered by a team of experienced GPs from Novum Health Partnership (Rushey Green), Honor Oak Practice and Amersham Vale Practice.
Each clinic is delivered by the same GP to provide continuity of care to the clients in order to provide consistency in the support provided and ensuring client engagement and confidence.
The service works closely with other services and stakeholders such as the following;
- Health Inclusion Team (homeless health nurses) ,
- drug & alcohol services,
- primary care mental health
- Rough Sleepers Outreach Team
- Homeless Health Inclusion Team
- and other key services to ensure that the clients have access to timely, good quality and appropriate primary care services for their physical and mental health needs.
Monthly multidisciplinary Team meetings are held to discuss complex cases and share learning.

Alcohol addiction - support for family and friends

New Direction provides an integrated treatment system for adults who have a problem with drug and alcohol use. The service works with people at all points of their treatment journey from engagement into services to recovery. We also provide support services to family and carers whose lives are affected by another’s drug and alcohol use. The open access service offers assessment 5 days a week.
We offer a variety of treatment services including:
- Assessment
- Open access and Brief Interventions
- Outreach and engagement services across Lewisham
- Opiate substitute prescribing and detoxification
- Keyworking
- Counselling – on hold
- Hospital and GP Liaison services for alcohol related problems
- Targeted Group work (Cannabis, Alcohol, Stimulant and Drugs Recovery Groups)
- Job clubs (with Job Centre Plus) – on hold
- Peer support
- Computer access and training – on hold
- Ear acupuncture and massage – on hold
- Family and carer support
- Referral to detoxification and residential rehab services
- Needle exchange
- Hepatitis testing and immunisation
- Street outreach services
- Home visits
- Assessment and referral to other agencies
- Gender specific groups – limited face to face groups at present due to COVID 19 so we are not running gender specific groups
- Dual diagnosis support
- Friends and family service
- Service user involvement group
- Computer Training – on hold
- NA, CA and SMART groups also work from our premises (not running due to COVID 19)
